Sample Assessment Tool Questionnaire

Each statement has two dimensions which must be rated. The first concerns how much conflict surrounds the statement among stakeholders in the business. The second concerns the performance achieved related to the statement. Rate each question or statement on a scale of 1 to 7 with 1 being a very poor rating and 7 being an excellent rating.
